Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: COWIEI5PD Site Location: WB 205 FREEWAY WEST OF GRANT LINE RD <br />Activity Summary <br />Activity Date <br />Recorded by <br />08/27/2021 Received update from Lead RENS Elena Manzo (EHD) who went to spill site after MURO <br />hours. Reviewed her narrative and photos in the Unit III > 2500 - Emergency <br />Response > 01 Pending Complaints folder. <br />08/27/2021 9:09 AM Called Brandon Espinosa (All Environmental) 209-648-8060 to let them <br />know that an area near the storm drain needed additional cleanup. Brandon asked <br />that I get there by 10:30 AM to verify cleanup. <br />Sent Brandon a photo of the area that needs additional cleanup via text. <br />9:27 AM Called Juan Armendarez (Caltrans) to ask how much absorbent was <br />used for the initial spill cleanup over the side of the freeway. Juan stated that he <br />requested that information from other Caltrans personnel, but they haven't gotten <br />back to him. <br />9:54 AM Left EHD. Arrived at spill site at 10:23 AM. <br />The area on the side of the freeway next to the storm drain that needed additional <br />cleanup was cleaned up with absorbent. The debris was removed from the storm <br />drain. The vegetation surrounding the storm drain outlet was dug out <br />approximately 2.5 X4 feet in area and between 10 - 12 inches deep. The <br />diesel-impacted concrete, storm drain, pipe, and outlet were then power-washed. <br />All rinsate was collected. All areas were cleaned up adequately, as no diesel odor <br />or oily sheen was detected. <br />- One partially-filled 55-gallon drum with approximately 30 gallons Non-RCRA <br />Hazardous waste diesel-contaminated liquid was generated. <br />- Two 55-gallon drums diesel contaminated solids was generated. <br />11:57 AM Returned to EHD. <br />Entered Narrative. <br />Generated Prop 65. <br />MURO <br />09/07/2021 Received a Hazardous Materials Incident Report #: 9266-2021-10485 via US Mail MURO <br />from Lieutenant Commander D. Wharry (Department of California Highway <br />Patrol). Read report. <br />09/08/2021 Generated photo log. MURO <br />09/22/2021 Complaint file review for abatement process. Noted items still needed to be printed MURO <br />on a post-it note on complaint file. <br />10/13/2021 Received a call from Ron with Bridge Port Insurance, Ron Herrygers MANZO <br /><rherrygers@windstream.net>, 803-917-7523, discussed billing issues, Ron <br />requested complaint copy when abated, he will provide waste disposal records as <br />soon as he has them available. <br />Received an email from Ron : "Thank you for the information you provided. I will <br />forward you a copy of the waste disposal documentation, as soon as it is provided <br />to me. Please consider this email as an official request for a copy of your Repor <br />after you have completed it. <br />". Sent a response email back indicating that I received his email. <br />10/20/2021 Printed remaining documents.
