At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle hazardous waste (HW) were properly trained. The following observations were made in the store's back storage and HW areas: - A Toxic HW tote, with accumulation start date 8/16/18, lacked the physical state on the HW label (IMG 1859). Audra Nepote, Store Manager, marked the physical state at the time of inspection (IMG 1860). - A CVS Health TENS Muscle Pain Relief, containing a control unit, a CR2032 battery, and a reusable electrode pad, was found marked as "Defective" inside the Toxic HW tote with accumulation start date 8/16/18 (IMG 1867-1868, 1870), however the item scanned as Universal Waste (IMG 1869). Audra removed the item from the Toxic HW tote and placed it inside a Universal Waste tote at the time of inspection. - A box of Franzia Cabernet Sauvignon, labeled as Flammable HW and appeared damaged on one side of the box (IMG 1872), was found not containerized on a shelf adjacent to the HW storage area (IMG 1871-1872). Audra stated that large boxed items of hazardous waste are typically not put into a HW tote, but rather a HW label is affixed to the box itself. - A turquoise colored USB cable with UPC 84113310089, was found marked as "Defect" in an open unlabeled cardboard box, and scanned as "SBT Item Markdown Not Allowed Return To Vendor Do Not Dispose in Dumpster" (IMG 1873, 1876-1878). When asked how this item would be managed, Audra stated it would be returned to the vendor. The following observations were made in the store's pharmacy: - The pharmacy's HW tote lacked the physical state on the HW label (IMG 1883). The staff pharmacist, Aarinola Ajayi, marked the physical state at the time of inspection. - An unlabeled open blue bin contained several unidentified loose pills (IMG 1884-1886). According to Aarinola, loose pills are placed into this blue bin until they are processed and placed into the pharmacy's HW tote every Sunday. The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. Provide proof of training to the EHD for employees whose responsibilities include hazardous waste. This is a Class II violation.
