When asked how this item would be managed, Audra stated it would be returned to the <br />vendor. <br />The following observations were made in the store's pharmacy: <br />- The pharmacy's HW tote lacked the physical state on the HW label (IMG 1883). The staff pharmacist, Aarinola <br />Ajayi, marked the physical state at the time of inspection. <br />- An unlabeled open blue bin contained several unidentified loose pills (IMG 1884-1886). According to Aarinola, <br />loose pills are placed into this blue bin until they are processed and placed into the pharmacy's HW tote every <br />Sunday. <br />All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br />information. <br />This is a Class II violation. <br />712 <br />CCR 66273.34 Failed to label universal waste to identify type of universal waste. <br />The following observations were made in the store's back storage and HW areas: <br />- A CVS Health TENS Muscle Pain Relief, containing a control unit, a CR2032 battery, and a reusable electrode <br />pad, was found marked as "Defective" inside the Toxic HW tote with accumulation start date 8/16/18 (IMG <br />1867-1868, 1870), however the item scanned as Universal Waste (IMG 1869). Audra removed the item from the <br />Toxic HW tote and placed it inside a Universal Waste tote at the time of inspection. <br />Universal waste shall be marked with the words "Universal Waste" and the type of waste (batteries, mercury <br />containing equipment, lamps, electronic devices, CRTs, CRT glass).
