Immediately
ensure all hazardous waste is contained in a closed hazardous waste container when not adding or removing waste.
This is a minor violation.
605
CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste.
The hazardous waste tote for toxic pharmaceuticals, located in the pharmacy, was missing the physical state and
contents. The pharmacists, Chao Vang, marked the physical state and contents on the container. This was
corrected on site. The hazardous waste tote for oxidizers, located in the hazardous waste storage area at the back
of the store, was missing the physical state. Dwayne Cavanas, Store Manager, marked the physical state. This was
corrected on site. Two StrongPak bags, one containing make-up and the other containing a bottle of shampoo, were
found unlabeled in a red "OTC' return basket at the front of the facility. All hazardous waste containers shall be
marked with the following information:
- the words "Hazardous Waste"
- name and address of generator
- hazardous properties
- physical state
- composition (contents)
- accumulation start date
Immediately place the two StrongPak bags containing the make-up and shampoo into properly labeled hazardous
waste containers and ensure that all hazardous waste containers are marked with all the required information.
This is a minor violation.
713
CCR 66273.35 Stored UW onsite greater than one year or was unable to demonstrate the time UW has been
stored.
A box containing and labeled as Universal Waste Bulbs lacked an accumulation start date. Universal waste may be
accumulated for no longer than one year from the date the universal waste was generated. The universal waste
handler shall be able to demonstrate the length of time that the universal waste has been accumulated. Since it
could not be demonstrated how long this waste was stored on site, immediately dispose of this waste and submit a
copy of the disposal receipt to the EHD.
This is a minor violation.
Overall Inspection Comments:
Notes:
- Waste streams observed: retail waste, pharmaceutical waste, and universal waste.
- A lip cream, Cream Licious (UPC:064967404770), was determined to be a hazardous waste, although the
"Zebra" scanner stated to dispose of the cream in the dumpster.
An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the
FA0002791 PR0531207 SCO01 08/14/2018
EHD 22-01 Rev. 09/22/16 Page 6 of 6 Small Quantity Hazardous Waste Generator OIR
