Immediately <br />ensure all hazardous waste is contained in a closed hazardous waste container when not adding or removing waste. <br />This is a minor violation. <br />605 <br />CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />The hazardous waste tote for toxic pharmaceuticals, located in the pharmacy, was missing the physical state and <br />contents. The pharmacists, Chao Vang, marked the physical state and contents on the container. This was <br />corrected on site. The hazardous waste tote for oxidizers, located in the hazardous waste storage area at the back <br />of the store, was missing the physical state. Dwayne Cavanas, Store Manager, marked the physical state. This was <br />corrected on site. Two StrongPak bags, one containing make-up and the other containing a bottle of shampoo, were <br />found unlabeled in a red "OTC' return basket at the front of the facility. All hazardous waste containers shall be <br />marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately place the two StrongPak bags containing the make-up and shampoo into properly labeled hazardous <br />waste containers and ensure that all hazardous waste containers are marked with all the required information. <br />This is a minor violation. <br />713 <br />CCR 66273.35 Stored UW onsite greater than one year or was unable to demonstrate the time UW has been <br />stored. <br />A box containing and labeled as Universal Waste Bulbs lacked an accumulation start date. Universal waste may be <br />accumulated for no longer than one year from the date the universal waste was generated. The universal waste <br />handler shall be able to demonstrate the length of time that the universal waste has been accumulated. Since it <br />could not be demonstrated how long this waste was stored on site, immediately dispose of this waste and submit a <br />copy of the disposal receipt to the EHD. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Notes: <br />- Waste streams observed: retail waste, pharmaceutical waste, and universal waste. <br />- A lip cream, Cream Licious (UPC:064967404770), was determined to be a hazardous waste, although the <br />"Zebra" scanner stated to dispose of the cream in the dumpster. <br />An inspection checklist was provided to the facility operator on the day of inspection.
