Please submit the results to the UST email for processing (UST@sjgov.org). All test
results, testing notifications and appointment requests should be submitted to EHD through the UST email.

I had some questions on the two Monitoring System Certification results that were conducted on 8/30/2022
1. Both Monitoring System Certification results for the top and bottom Veeder-Root console have “no” marked off
in Section 5. for “All monitoring equipment is operational per manufacturer’s specifications”. For every item
marked as a “no” or “fail” a proposed remedy should be noted in Section 9. In Section 9 for both Monitoring
System Certification forms what the Veeder-Root console monitors is listed and does not address the “no”
marked.
2. The alarm history for both Veeder-Root consoles are from 9/1/2022 and not on 8/30/2022, the date of the
testing. Is there an alarm history for 8/30/2022?
The alarm history submitted (printed 9/1/2022) has missing alarms to show that these sensors were
tested on 8/30/2022. I have a table of the missing alarms below:
Sensor
# Sensor Name Missing Alarms
Date Alarm Type
Q4 GAS ADDITIVE 8/30/2022 PLLD SHUT DOWN
S23 PREMIUM VENT VAC 8/30/2022 NO VACUUM
HIGH LIQUID
S25 GAS ADD1.
