A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> COSTCO WHOLESALE#658 3250 W GRANT LINE RD,TRACY November 08, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 319 CCR 2635, 2637.2,2665, 2712 Failure to comply with overfill prevention equipment requirements. <br /> OBSERVATION: The 91 tank did not have a required overfill prevention system prior to receiving fuel in the 91 tank. <br /> Fuel was deposited into the 91 tank on 8/29/22,without conducting required testing of the overfill prevention system. <br /> Functionality testing of the flapper overfill for the 91 premium tank was observed on 8/30/22. <br /> REGULATION GUIDANCE:All underground storage tanks shall be equipped with an overfill prevention system that <br /> does not allow for manual override and meets one of the following requirements: <br /> 1. Alert the transfer operator when the tank is 90 percent full by restricting the flow into the tank or triggering an <br /> audible and visual alarm; or <br /> 2. Restrict delivery of flow to the tank at least 30 minutes before the tank overfills, provided the restriction occurs <br /> when the tank is filled to no more than 95 percent of capacity; and activate an audible alarm at least five minutes <br /> before the tank overfills; or <br /> 3. Provide positive shut-off of flow to the tank when the tank is filled to no more than 95 percent of capacity;or, <br /> 4. Provide positive shut-off of flow to the tank so that none of the fittings located on the top of the tank are <br /> exposed to product due to overfilling. <br /> CORRECTIVE ACTION: Ensure all operated UST systems have approved overfill prevention system going forward. <br /> This is a Class II violation. <br /> 320 CCR 2635(c)(1)Overfill prevention was overridden. <br /> OBSERVATION: The mechanical overfill prevention device on the 91 UST was overridden by sending fuel directly <br /> into the body of the tank, as per Jones Covey Group. <br /> REGULATION GUIDANCE:All underground storage tanks shall be equipped with an overfill prevention device that <br /> does not allow for manual override and either shuts off or restricts flow into the tank. <br /> CORRECTIVE ACTION: Take all necessary precautions to prevent overfill prevention devices from being overriden <br /> in the future. <br /> This is a Class I violation. <br /> FA0013810 PR0518288 SCO01 11/08/2022 <br /> EHD 23-01 Rev.12/06/2021 Page 7 of 8 Underground Storage Tank Program OIR <br /> 1868 E.
