The following was found in the store's back storage area: <br /> -Eight containers of Oxi Clean laundry detergent,one leaking and another with a broken cap,were found under an <br /> empty grey tote in the mop sink(see photos: IMG 1338-1341). <br /> -Two damaged boxes of wine were found on a shelf adjacent the HW storage area,not bagged, nor containerized, <br /> and one of the boxes was not labeled,while the other box was labeled as Flammable HW(see IMG 1333&1335). <br /> -Adjacent to the HW totes,were six open unlabeled white cardboard boxes containing un-bagged HW(IMG 1337). <br /> According to the store's stock clerk receiver, Ray Koker, he is the only employee that processes the HW about <br /> every other day once he has time do so. <br /> -A Kiss Ready-To-Wear Gel nail package was found damaged in an open unlabeled white cardboard box,and <br /> scanned as Toxic HW(IMG 1345-1346). <br /> -A CVS Health Vapor Steam Cough Suppressant Camphor was found in an open unlabeled white cardboard box, <br /> and scanned as Flammable HW(IMG 1347-1348). <br /> -A damaged Gold Bond medicated moisturizer,labeled"Damaged,"was found in an open unlabeled white <br /> cardboard box,and scanned as Toxic HW(IMG 1349-1351). <br /> -An expired Children's Zyrtec Allergy Grape Syrup was found in an open unlabeled white cardboard box,and <br /> scanned as Toxic HW(IMG 1352-1354). <br /> -An Olay Whip Shea Butter Body Wash was found with a broken spray nozzle was found in an open unlabeled <br /> white cardboard box, and scanned as Flammable HW(IMG 1355-1356). <br /> -A La Roche-Posay Anthelios 50 Ultra Light Sunscreen was found in an open unlabeled white cardboard box,and <br /> scanned as Toxic HW(IMG 1359-1360). <br /> -A bottle of Herbal Essences shampoo was found leaking in a StrongPak bag in an open white cardboard box <br /> labeled as"Large Items(defects)"(IMG 1362-1363). Ray did not know what happened to the debris used to <br /> clean-up the shampoo leakage, nor did he know who conducted the clean-up. <br /> -A bottle of Creme of Nature Argan Oil spray was found leaking in a StrongPak bag without absorbent, in an open <br /> white cardboard box labeled as"Large Items(defects)"(IMG 1362& 1364). <br /> -A container of Tide laundry detergent and contaminated absorbent was found in a StrongPak bag on a bottom <br /> shelf in the storage room,but was unlabeled and not containerized(IMG 1365-1366). <br /> All of the above items were identified as waste by Ray Koker. <br /> The following was found in the pharmacy: <br /> -A bottle of red liquid medicine was found leaking in a StrongPak bag without absorbent in the pharmacy's HW tote <br /> (IMG 1376). <br /> -Empty Warfarin bottles,other types of pharmaceutical bottles,and loose pills were found contained in the same <br /> StrongPak bags inside the HW tote(IMG 1374-1375, 1377-1378). <br /> -Loose pills were found bagged and labeled with the NDC number, but without the name of the drug inside the HW <br /> tote(IMG 1379).When asked how Stericycle would know whether or not any of the loose pills were Warfarin or <br /> Coumadin, Shuchi Gupta,pharmacy manager,stated she guessed Stericycle would not know since the NDC <br /> number would have to first be found in the pharmacy computer system. <br /> The following was found in a grey tote labeled as"Defects Empty Packages"at the front of the store behind the <br /> registers(IMG 1388-1389): <br /> -A Condition 3-in-1 Maximum Hold Mousse,scanned as Flammable HW(IMG 1381-1382). <br /> -A Wunderbrow Dual Precision Brow Liner,scanned as Toxic HW(IMG 1383-1385). <br /> -A Truly Yours Dark Circle Concealer,scanned as Toxic HW(IMG 1386-1387). <br /> All of the above items were identified by Ray Koker as waste that had not been processed yet. <br /> FA0002003 PR0537749 SC001 08/24/2018 <br /> 1-HD 22-01 Rev.09/22/16 Page 4 of 10 Small Quantity Hazardous Waste Generator OIR <br />
