Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />December 19, 2018 <br /> Facility Address: <br /> 5756 PACIFIC AVE, STOCKTON <br /> Facility Name: <br /> SALLY BEAUTY SUPPLY #744 <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 111 CCR 66262.42(a)(c)(d) Failed to comply with uniform hazardous waste manifest exception requirements. <br />- A copy of uniform manifest 015886817JJK (10/24/16) for the disposal of hazardous waste was not found on site. <br />Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br />used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br />Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit a <br />copy of manifest 015886817JJK (10/24/16) to the EHD. <br />- Manifest number 019594604JJK (10/26/18) (IMG 3227) was found without a signed off copy from the destination <br />facility. <br />Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three <br />years and have them readily available for review. If the generator did not receive a copy of the manifest with the <br />handwritten signature of the owner or operator of the facility to which the generator’s waste was submitted within 60 <br />days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the <br />missing manifest, with some indication that the generator has not received confirmation of delivery. This information <br />shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. <br />Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br />This is a Class II violation. <br /> 119 HSC 25404(e)(4) Failed to report program data in the California Environmental Reporting System (CERS). <br />The facility failed to report the program data eletronically on the California Environmental Reporting System (CERS). <br />The owner or operator shall declare that the facility is a hazardous waste generator and enter the facility EPA ID <br />number in the business activities section in CERS. <br />Log on to CERS at http://cers.calepa.ca.gov to make a hazardous waste submittal on the CERS website. Starting <br />September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). According to Priscilla Valenzuela, store manager, electronics placed in this bin are sent back to the <br />manufacturer.
