Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />December 19, 2018 <br /> Facility Address: <br /> 5756 PACIFIC AVE, STOCKTON <br /> Facility Name: <br /> SALLY BEAUTY SUPPLY #744 <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle <br />hazardous waste were properly trained. <br />Based on the types of violations observed, the facility employees are not trained on proper hazardous waste <br />handling or the training provided has been inadequate. <br />- The Waste Management Training DVD Sign Off sheet was found blank in the Sally Beauty Holdings Hazardous <br />Waste Training Guide binder at the time of inspection (see photo IMG 3231). According to Priscilla, store employees <br />have viewed the video for How to Categorize Products (California) (see photo IMG 3232), however she did not have <br />them sign off on the training sheet. <br />The following observations were made in the store's back storage room at the time of inspection: <br />- The following items were found in the Corrosive Acids Hazardous Waste (HW) tote (see photo IMG 3153): <br /> - A Design Freedom Neutralizer (for hair), which listed hydrogen peroxide as an ingredient (IMG 3154-5156). <br />According to the Sally Beauty Holdings Hazardous Materials Management Poster for California (The Poster), found <br />in the facility’s Ipad within an application called Athena (IMG 3151-3152) and posted on a wall adjacent the <br />hazardous waste storage area (IMG 3146), items containing hydrogen peroxide shall be placed in the Oxidizer HW <br />tote. Priscilla placed the item in the Oxidizer HW tote at the time of inspection. <br /> - An ORS Olive Oil Built-In Protection New Growth No-Lye Hair Relaxer Base, which was labeled as containing <br />alkali (IMG 3157-3158). According to The Poster, hair relaxer (lye & no-lye) with alkali shall be placed in the <br />Corrosive Bases HW tote. Priscilla placed the item in the Corrosive Bases HW tote at the time of inspection. <br /> - A Just For Me Texture Softener Crème Children’s No-lye Texturizer, which listed calcium hydroxide as an <br />ingredient (IMG 3159-3161). According to The Poster, hair relaxer (lye & no-lye) with calcium hydroxide shall be <br />placed in the Corrosive Bases HW tote. Priscilla placed the item in the Corrosive Bases HW tote at the time of <br />inspection. <br /> - A Soft & Beautiful Botanicals No-Lye Conditioning Relaxer Crème (IMG 3162). According to The Poster lye & <br />no-lye hair relaxers shall be placed in the Corrosive Bases HW tote. Priscilla placed the item in the Corrosive Bases <br />HW tote at the time of inspection. <br /> - A Lusters S Curl Texturizer Wave & Curl Crème, which listed sodium hydroxide as an ingredient (IMG <br />3163-3164). According to The Poster, hair relaxer with sodium hydroxide shall be placed in the Corrosive Bases <br />HW tote. Priscilla placed the item in the Corrosive Bases HW tote at the time of inspection. <br /> - A TCB No Base Crème Hair Relaxer, which listed sodium hydroxide as an ingredient (IMG 3165-3166). <br />According to The Poster, hair relaxer with sodium hydroxide shall be placed in the Corrosive Bases HW tote. <br />Priscilla placed the item in the Corrosive Bases HW tote at the time of inspection. <br />- The following items were found in the Oxidizers HW tote (IMG 3167): <br /> - Four tubes of Ion Semi-Permanent Crème Hair Color (IMG 3169-3170). According to The Poster, <br />semi-permanent hair color shall be placed in the State Toxic HW tote. Priscilla placed the item in the State Toxic <br />HW tote at the time of inspection. <br /> - Four containers of Silk Elements Semi-Permanent Hair color (IMG 3169, 3171). According to The Poster, <br />semi-permanent hair color shall be placed in the State Toxic HW tote. Priscilla placed the item in the State Toxic <br />HW tote at the time of inspection. <br /> - A Tish & Snooky’s Manic Panic NYC Semi-Permanent Hair Color Cream (IMG 3169, 3172). According to The <br />Poster, semi-permanent hair color shall be placed in the State Toxic HW tote. Priscilla placed the item in the State <br />Toxic HW tote at the time of inspection. <br /> - Four containers of Clairol Professional Beautiful Collection Semi-Permanent Color (IMG 3169, 3173).
