Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />December 27, 2018 <br /> Facility Address: <br /> 4339 E MORADA LN, STOCKTON <br /> Facility Name: <br /> SALLY BEAUTY SUPPLY #10530 <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />Based on the types of violations observed, the facility employees are not trained on proper hazardous waste (HW) <br />handling or the training provided has been inadequate. <br />- When asked what type of training she and the rest of the store employees have received, Zulema Ojeda, the store <br />manager, stated she was told by her manager to watch the Sally Beauty Hazardous Waste Training DVD (see photo <br />IMG 3260) at the beginning of the year, which she and the other employees watched at the beginning of 2018 for <br />the first time. Zulema stated she was told by her manager that the DVD is to be viewed by all the employees at least <br />once each year. Zulema stated that the DVD has been in the onsite hazardous waste binder since she started <br />working at this facility about 2-3 years ago. A Waste Management Training DVD Sign Off sheet was observed on <br />site, which was signed off by Zulema Ojeda, Guadalupe Morales, Leslie Sanchez, and Priscilla Williams on 1/8/18 <br />(IMG 3285). Zulema did not have any other employee training records at the time of inspection. <br />- When asked if she has access to the Athena application on the store's Ipad, Zulema stated she can open the <br />application, but the hazardous waste training material does not load. Zulema stated she and the other store <br />employees have not viewed the Sally Beauty Hazardous Waste Training Video and the Sally Beauty How to <br />Categorize Products from the Athena application (IMG 3264). <br />- When asked how employees determine which HW tote [Corrosive Acids, Corrosive Bases, Oxidizers, Toxic & <br />State Regulated Waste (State Toxic), or Aerosols, Flammables, & Combustibles (Flammables)] an item should be <br />placed in, Zulema stated she and the rest of the employees use the Sally Beauty Hazardous Materials Management <br />Poster (The Poster), posted on a wall adjacent the HW storage area (IMG 3259 and 3283), and a document titled <br />Hazardous Waste Information Guide (IMG 3280-3282). Zulema stated when she is unsure which HW tote an item <br />should be placed in, she will call other Sally Beauty Supply store managers to ask which tote they think the item <br />should be placed in, and she will place the item in the HW tote that she and the other store manager agree to. <br />Zulema stated she calls other store managers, since her district manager, Hilary Mcgahey, often cannot be reached <br />via telephone. Zulema stated she has never called the Sally Beauty Hotline number, written on The Poster. <br />- When asked if she uses the Sally Beauty Safety Data Sheet (SDS) application on the store's Ipad, Zulema stated <br />she had not used the application until last week, when she was told by her manager to use the application to <br />determine which HW tote an item should be placed in. Zulema stated she and the other employees are still learning <br />how to use the application, and that when she tried entering the SKU numbers of several items into the SDS <br />application, no information was found. <br />- The following was observed in the facility's hazardous waste (HW) storage area: <br /> - a State Toxic HW tote contained a Gigi All Purpose Honee hair removal wax, however the HW label on the tote <br />did not include hair waxes under "Contents" (IMG 3300-3301). <br /> - a Corrosive Bases HW tote contained an Ion Color Brilliance Permanent hair color, however the HW label on the <br />tote did not include hair color under "Contents" (IMG 3303-3304). <br />- The following HW totes were found with broken/cracked lids: <br /> - a Flammables HW tote with accumulation start date 12/27/18 and contained a Beyond The Zone (BTZ) hairspray <br />labeled as flammable (IMG 3276-3278). <br /> - a Corrosive Acids HW tote with accumulation start date 8/17/18 (IMG 3269-3272) and an Oxidizers HW tote with <br />accumulation start date 8/17/18 (IMG 3273-3274), although these totes did not contain any HW waste items at the <br />time of inspection. <br /> Zulema stated she received three new lids today to replace the broken lids (IMG 3289). <br /> <br />Page 5 of 7 <br />FA0023588 PR0541186 SC001 12/27/2018 <br />EHD 22-01 Rev. 09/22/16 Small Quantity Hazardous Waste Generator OIR
