STATE OF CALIFORNIA California Integrated Waste <br /> CIWMB 3 (Rev. 4/04) Management Board <br /> INSPECTION REPORT ADDENDUM PAGE 2 OF 2 <br /> AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br /> SITE NUMBER: 39-AA-0004 INSPECTOR: Garrett Backus <br /> SITE NAME: Foothill Landfill <br /> RECEIVED BY <br /> INSPECTION DATE: November 21, 2006 OPERATOR ;-Z,— <br /> COMMENTS: <br /> A routine inspection was done on this day. <br /> 1) Note: On July 26, 2006, an emergency waiver of permit conditions was granted to Foothill Landfill by the <br /> Environmental Health Department due to the extreme heat this summer and the related increase in livestock <br /> mortality. The permit conditions which may be waived are: daily peak tonnage, permitted traffic volume, and <br /> hours of operation. The emergency waiver is in effect for no more than 120 days and expires on November 23, <br /> 2006. <br /> Submit a written report to the Environmental Health Department(EHD) which includes the amount of disaster <br /> debris received at the facility during the final 30 days of the waiver period(October 25 —November 23, 2006). <br /> Submit a report every 90 days if disaster-related waste is still being processed or stored at the facility. Submit a <br /> hard copy of the report on letterhead. The T14 CCR 17210.5, "Reporting Requirements for a Solid Waste <br /> Operator," describes the information to be included in the report. <br /> 2) Note: Discussed the site Winterization Plan with the operator. Complete the Plan and send to the EHD for <br /> review and filing. <br /> 3) Note: Reviewed daily load check records. Records were complete and up-to-date. <br /> 4) Note: Reviewed tonnage and traffic records. Records were within the permit limits. <br /> 5) Note: The site employees and transfer truck drivers were wearing personal protective equipment during the <br /> inspection, including hard hats and safety vests. <br /> 3) Note: Site Improvement Project(SIP)—Work on module "P'was in progress during the inspection. <br /> INSPECTOR SIGNATURE: DATE: Z O� <br />
