SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUN r <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ALPHA ENTERPRISE CORP 23 Maxwell St, Lodi January 10, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The following hazardous materials were found on site and have not been reported to CERS: <br /> -approximately 155 gallons of Waxie W-400 floor dripper was observed in the southwest storage room <br /> -approximately 166 gallons of Pioneer Eclipse Formula X-treme was observed in the southwest storage room and <br /> near the battery storage area <br /> -approximately 450 gallons of Pioneer Eclipse CIS Top Shield was observed near the battery storage area <br /> -approximately 100 gallons of Pioneer Eclipse HD Degreaser was observed near the battery storage area <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> ---------------------- <br /> ---------------------- <br /> OBSERVATION: The Hazardous Materials Inventory forms for the waste oil and propane contain obsolete hazard <br /> categories. <br /> REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete.
