RECEIVED The following is an itemized list of underground storage tank violatidWAS 1242p not been addressed for DINO MART as of April28 , 2022 . ENVIRONMENTAL HEA TH Open violations from January 06 , 2022 inspecti ARTMENt Violation #122 - Failure to implement corrections and submit written response within 30 days of receiving the report . OBSERVATION An inspection was last done on 04/26/2021 and an inspection report was issued identifying information to be submitted to bring this site into compliance . This information was required to be submitted by 05/26/2021 . This information was received on 09/ 17/2021 four months late resulting in a non -compliant status for this facility . REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to implement the corrections specified in the inspection report and comply with permit conditions . The corrective action shall include all of the recommendations made in the report. The operator shall submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken . CORRECTIVE ACTION : Ensure that a written response documenting corrective actions taken or proposed is submitted and all corrections implemented within 30 days of receiving an inspection report. ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #203 - Failure to maintain monitoring records for 36 months and maintenance records for 60 months. OBSERVATION Maintenance and monitoring records for the last three years were not found on site . - The 2020 monitoring system certification inspection report was not on site during the inspection . REGULATION GUIDANCE: These records shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; ( 3 ) results of any visual observations ; (4 ) results of sample analysis performed a lab or in the field ; ( 5 ) logs of all readings of gauges or other monitoring equipment, ground water elevations , or other test results ; (6 ) results of inventory readings and reconciliations .
