The following is an itemized list of underground storage tank violations that have not <br />been addressed for J & L MARKET as of August 05, 2022. <br />Open violations from May 23, 2022 inspection <br />Violation #109 - Failure to submit "Designated Underground Storage Tank Operator Identification Form" <br />within 30 days. <br />OBSERVATION: A new designated operator, Amy Winkler, conducted an inspection on 2/23/2022 and notification <br />was not provided to the EHD within 30 days of the change. <br />REGULATION GUIDANCE: The owner or operator shall submit a "Designated Underground Storage Tank Operator <br />Identification Form" no later than 30 days after any change in the designated UST operator(s). <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #208 - Failure to comply with the designated operator inspection requirements. <br />OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br />reports. The incorrect and missing information included the following from the inspection reports: <br />-Section 9 "UST System Inspection" was missing the "Spill Containment ID" for the diesel tank <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Open violations from May 19, 2021 inspection <br />Page 1 of 3 <br />
