Sally Beauty Supply#752, 678 N Wilson Way, Stockton, CA Record ID PR0540934 <br /> 10/16/2018 <br /> PHOTO# 1 <br /> The hazardous waste (HW)items listed below were found in HW totes with labels that did not include these items as contents of the totes, nor were the <br /> items on the Sally Beauty Holdings Inc. "Hazardous Material Management" poster. When asked how a waste determination is made for such items, Tina <br /> Flores, the assistant store manager, stated she either asked the store manager, Patricia Brewer, or she would guess which HW tote the item should be <br /> placed in. Tina did not appear familiar with the Sally Beauty Hot Line number listed on the "Hazardous Material Management poster to call if questions <br /> came up. Also, Tina did not know about the Sally Beauty Application, which provides item Safety Data Sheets (SDS)to make waste determinations, until <br /> she called her District Manager, Hillary Mcgahey. At this point, Tina still was not clear on information needed to help make a waste determination from the <br /> SDS. Towards the end of the inspection she told us there is a new poster to help identify waste streams that apparently was sent to the store, but they had <br /> not received. <br /> -Two containers of Gigi Hemp Wax, one of which was leaking and without absorbent, were found in the Corrosive Acids HW tote. According to Sally <br /> Beauty Holding's "Guidelines for Accumulating Hazardous Waste" poster, absorbent must be placed in the bag with a leaking container. <br /> -A container of GIGI Sensitive Tweezeless Wax was found in the Oxidizers HW Tote. <br /> -A leaking bottle of Protein Color Filler Primer Corrector Refresher was found without absorbent in the Corrosive Bases HW tote. <br /> -A bottle of Blond Brilliance Perfect Blond Instant Tone Mask was found in the Corrosive Bases HW tote. This item was found in a bag which also <br /> contained three used tubes of Loreal Chroma True Red's permanent hair color. <br /> -A container of Beyond The Zone Demented Styling Goo and a container of Barbury's By Sibel Pomade were found in the same bag inside the <br /> Aerosols, Flammables &Combustibles HW tote. <br /> f <br /> IQ{IW IMf? <br /> �V A, <br /> It .RAIN AT ALL TIM&&- <br /> c <br /> Elianna Florido, REHS Page 1 of 58 <br />
