<br /> Although Employee Hazardous Waste Training Logs were reviewed at the time of inspection (IMG 2156), it could
<br /> not be demonstrated (not necessarily documented)that employees who handle hazardous waste were properly
<br /> trained.The following was observed at the facility's hazardous waste(HW)storage area:
<br /> -Three out of the five hazardous waste(HW)totes that were observed did not have the name of the facility
<br /> generator on the HW label (see IMG 2118, 2131, and 2151). Janielle Bauer, the Store Manager, stated she was not
<br /> aware that the name of the facility/store was required on a hazardous waste label.
<br /> -Although all five of the HW tote labels that were observed listed the words"solid"and "liquid"for the physical state,
<br /> the labels were not marked to indicate whether the current contents in the totes were either solids, liquids, or both
<br /> (IMG 2118, 2120,2130, 2131, and 2151).
<br /> -Although all five of the HW tote labels listed various items for each category of HW,the labels were not marked to
<br /> indicate the current contents in each tote and there were several items in the HW totes that were not listed as
<br /> contents on the labels(IMG 2118, 2120, 2130, 2131, and 2151).
<br /> -A container of Avlon Affirm Conditioning Relaxer System Creme Relaxer was found bagged with absorbent pads
<br /> inside a Corrosive Acids HW tote(IMG 2127-2130). However, according to a Sally Beauty Holdings, Inc"Hazardous
<br /> Material Management"poster posted directly above the HW storage area, hair relaxers are listed as corrosive bases
<br /> (IMG 2126). Janielle stated the wrong label was placed on the tote, and replaced the corrosive acids HW label with
<br /> a corrosive bases HW label at the time of inspection.
<br /> -An It's A 10 Miracle Super Hold Finishing Hair Spray was found bagged inside an Oxidizers HW tote (IMG
<br /> 2131-2132). However, according to the"Hazardous Material Management"poster, hair sprays are listed under
<br /> flammable gases, liquids, and solids.
<br /> -A Mizani Intense Moisturizing Mask for hair was found wrapped in absorbent pads, bagged, and inside an
<br /> Oxidizers HW tote(IMG 2131, 2133-2134). However, according to the"Hazardous Material Management" poster,
<br /> hair moisturizing masks did not correlate with any of the items listed under oxidizers. Janielle stated she would call
<br /> the hot line phone number written on the poster to find out which HW tote the moisturizing mask should be placed
<br /> into.
<br /> -A Vero K-Pak Color permanent cream color was found bagged and inside an Oxidizers HW tote(IMG 2131,
<br /> 2138-2129). However, according to the"Hazardous Material Management"poster, permanent hair color is listed
<br /> under corrosives bases.
<br /> -A Framcolor Bold hair coloring cream was found bagged and inside an Aerosols, Flammables, &Combustibles
<br /> HW tote(IMG 2140-2141). However, according to the"Hazardous Material Management"poster, semi-permanent
<br /> hair color is listed under state toxics.
<br /> -Two bottles of Starpro Quick Dry Resin were found bagged and inside an Aerosols, Flammables, &Combustibles
<br /> HW tote(IMG 2140, 2142-2144). However,when asked why this item was placed in the Aerosols, Flammables, &
<br /> Combustibles HW tote,Janielle stated that she thinks the item is flammable, but she was not entirely sure.Janielle
<br /> stated she would call the hotline phone number on the Hazardous Material Management poster to verify which tote
<br /> the item should be placed in.
<br /> -Spillage from a single bagged container of Joico Color Endure Shampoo was found inside a Toxics HW tote with
<br /> accumulation start date 9/11/18(IMG 2145-2147, 2151).According to a Sally Beauty Holdings, Inc"Guidelines For
<br /> Accumulating Hazardous Waste"poster posted directly above the HW storage area, if a container is leaking,
<br /> absorbent must be placed in the bag, and the container must be double bagged (IMG 2148). Janielle cleaned up the
<br /> spillage using absorbent, placed the absorbent inside the bag with the container, and double bagged the container
<br /> at the time of inspection.
<br /> -A Nioxin Bodifying Foam (for hair)aerosol can was found in a Toxic HW tote with accumulation start date 9/11/18
<br /> (IMG 2150-2151).
<br /> -A Paul Mitchell The Demi demi-permanent hair color was found wrapped in absorbent pads, bagged, and inside a
<br /> Toxic HW tote with accumulation start date 9/11/18 (IMG 2151, 2153-2154). However, according to the"Hazardous
