Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Date: <br />January 13, 2022 <br /> Facility Address: <br /> 15600 S HARLAN RD, LATHROP <br /> Facility Name: <br /> JOES TRAVEL PLAZA <br />Other Violations <br /> 2010 □ V □ R □ COSUnlisted General violation See below <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 109 CCR 2715(a)(1)(B), 2715(a)(3) Failure to submit "Designated Underground Storage Tank Operator Identification <br />Form" within 30 days. <br />OBSERVATION: The Underground Storage Tank operator identification Form was not found in CERS. <br />REGULATION GUIDANCE: The owner or operator shall submit a "Designated Underground Storage Tank Operator <br />Identification Form" no later than 30 days after any change in the designated UST operator(s). <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 02/12/2022. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($152).
