<br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW)
<br /> handling or the training provided has been inadequate.
<br /> The following observations were made in the store's back storage room at the time of inspection:
<br /> -An Employee Hazardous Waste Training Log sheet was observed containing employee names,titles, signatures,
<br /> and dates(see photo IMG 3574).According to the assistant store manager, Jeanette Rios, her signature, as well as
<br /> the signature for Chandra Cullum, dated 10/24/18 and 10/28/18, respectively, indicate that they have watched the
<br /> HW training videos on the Athena application on the store's iPad (IMG 3572).Tiffany Chacon's,the store manager,
<br /> signature is dated 6/20/18, and Alexa Didio's, sales associate, signature is dated 8/13/18. Jeanette stated she is
<br /> unsure if Tiffany and Alexa have watched the training videos on Athena. Jeanette stated Alexa's last day of work is
<br /> 1/12/19. Jeanette stated the other employees listed on the log, Katherine S., Leslie Knott, Morgan Daniels, and
<br /> Arturo Serrano, are former employees that no longer work at the store.There were no other training logs available
<br /> at the time of inspection.
<br /> -When asked how employees determine which HW tote a damaged/returned item should be placed in,Jeanette
<br /> stated employees refer to the Sally Beauty Holdings, Inc. Hazardous Material Management Poster(The Poster)
<br /> (IMG 3551-3552)posted on the wall above the HW storage area and the Sally Beauty Safety Data Sheet(SDS)
<br /> application on the store's iPad. Jeanette stated if an employee is still unable to determine which tote an item should
<br /> be placed in, after using The Poster and the SDS application, employees call other Cosmoprof stores to find out
<br /> which tote the item is placed into at that store. Jeanette stated she has not called the Sally Beauty Hotline number
<br /> listed on The Poster.
<br /> -A Sexy Hair Powder Play volumizing and texturizing hair powder was found in a Aerosols, Flammables, &
<br /> Combustibles(Flammables) HW tote(IMG 3554, 3558-3559).According to the item's SDS,this item is not
<br /> flammable(IMG 3560-3561). Jeanette placed the item into the State Toxic HW tote.
<br /> -A leaking container of Aquage color protecting shampoo was found double-bagged in the State Toxic HW tote
<br /> (IMG 3566-3567).According to the Guidelines for Accumulation Hazardous Waste poster, posted above the HW
<br /> storage area, and the Sally Beauty Holdings, Inc. Hazardous Waste Training Guide, if a container is leaking,
<br /> absorbent must be placed in the bag and the container must be double bagged (IMG 3551, 3553, 3580-3581).
<br /> -Two leaking containers of Framesi Color Lover Dynamic Blonde Shampoo were found single-bagged and without
<br /> absorbent in the State Toxic HW tote (IMG 3568).According to the Guidelines for Accumulation Hazardous Waste
<br /> poster, posted above the HW storage area, and the Sally Beauty Holdings, Inc. Hazardous Waste Training Guide, if
<br /> a container is leaking, absorbent must be placed in the bag and the container must be double bagged (IMG 3551,
<br /> 3553, 3580-3581).
<br /> -The Corrosive Bases HW tote label listed "examples"which did not include the type of item contained in the tote,
<br /> which was a Wella Professionals Koleston Perfect Permanent Creme Hair Color(IMG 3569-3571).
<br /> All of the items listed above were identified as waste by Jeanette at the time of inspection.
<br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper
<br /> waste handling and emergency procedures. Provide proof of training and training materials to the EHD for
<br /> employees whose responsibilities include hazardous waste.
