Timeln: 1:16 pm <br /> r <br /> Time Out: 1:40 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: GOLDEN OX Date: 01/10/2022 <br /> Address: 410 W KETTLEMAN LN, 95240 <br /> Owner/Operator: PATRONAS, IOANNIS E Telephone: (209)333-0146 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00054581 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES EMPLOYEES DO NOT WEAR FACE MASKS, SILVERWARE BY THE COOKS BEING USED <br /> OVER AND OVER AGAIN BEFORE BEING PROPERLY SANITIZED, COCKROACHES IN THE KITCHEN,AND OWNER <br /> HAS REQUIRED WORKERS TO COME IN THAT HAVE BEEN IN CONTACT WITH PEOPLE THAT TESTED POSITIVE <br /> FOR COVID-19 AND THE EMPLOYEE HAS THE EXACT SAME SYMPTOMS OF THOSE WHO TESTED POSITIVE. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe.
