3E Company's SDS service allows colleagues to receive live assistance 24-7-365 for all <br /> SDS requests. <br /> If an SDS is needed for spill response or poison/exposure information, the colleague should <br /> contact the CVS Health Safety Hotline. Upon initial contact, the colleague will be asked if <br /> he or she has a medical emergency or chemical spill. It is important for colleagues to <br /> identify if either of these situations is applicable. If neither is applicable, the caller will be <br /> asked for the following information to complete the SDS request process: <br /> • Callers facility <br /> • Name <br /> • Phone number <br /> • Fax number <br /> • Chemical/Product full label name and any applicable part numbers <br /> • Manufacturer <br /> 3E Company will deliver the requested document to the fax number specified and provide <br /> p Y <br /> the caller with his or her request number. This request number can be utilized for any <br /> follow-up activities related to this particular request.
