Rite Aid #6005, 1245 W Yosemite Ave, Manteca, CA Record ID PR0536077 <br /> 12/04/2018 <br /> PHOTO#4 <br /> The following pharmaceuticals were found in an open blue bin labeled "Weekly StrongPak Staging Bin—Amber Vials—Loose Pills" (IMG 4855,4868): <br /> -A StrongPak Hazardous Waste bag containing three smaller StrongPak - Four small bags, labeled as"Venlafaxine HCL 75 MG Tablet,""CVS Ibuprofen <br /> Hazardous Waste bags, each bag containing loose pills of a different type(IMG 200 MG Caplet,""Anastrozole 1 MG Tablet,"and"Klonopin 0.5 MG Tablet," each <br /> 4857). containing loose or broken pills. <br /> -A bag of used shringrix vials (IMG 4858). -A StrongPak Waste bag containing smaller bags which contained different <br /> -An aerosol can of Dermoplast Pain Relieving Spray(IMG 4859-4860). types of loose pills(IMG 4864). <br /> According to Hanh Pham,the pharmacy manager,the spray was returned to the -A StrongPak Waste bag containing a Boostrix TDAP Vaccine Syringe(IMG <br /> pharmacy by a patient.
