OBSERVATION: The designated operator failed to check and note the correct test dates for the Overfill Prevention Equipment Inspection on the designated operator inspection reports. On the 10/8/21, 7/16/21,4/23/21, and 3/25/21 designated operator reports,the designated operator incorrectly noted the Overfill Prevention Equipment Inspection date as 2/12/21. On the 5/20/21 designated operator report,the Overfill Prevention Equipment Inspection date was incorrectly noted as 1/2/20.The correct date for the Overfill Prevention Equipment Inspection is 3/12/21. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 304 CCR 2631(a); HSC 25290.1(c) Failure to construct, operate, and maintain primary containment as product-tight. OBSERVATION:Approximately 2 cups of diesel was found on the far left side of UDC 1/2 when facing pump 2, indicating a leak in the primary containment. REGULATION GUIDANCE:All primary containment for the UST system must be product tight. CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the failed component(s), under permit and inspection of the EHD if necessary. This is a repeat violation, Class II. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 3/13/2022. To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
