S0 JOA U 1 Environmental Health Department <br /> U ;� i Y I <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FIELD TOWER SYSTEMS (KS SOLUTIONS) 28135 E COPPEROPOLIS RD, FARMINGTON February 10, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 2 HSC 25505, 25508(a)(1)Failed to initially complete and electronically submit a hazardous materials business plan <br /> (HMBP). <br /> OBSERVATION:A complete Hazardous Materials Business Plan was not submitted electronically to the California <br /> Environmental Reporting System (CERS). <br /> REGULATION GUIDANCE:As of January 1, 2013, all businesses were required to submit all business plans <br /> electronically. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. <br /> OBSERVATION: The Business Owner/Operator Identification form was not submitted in CERS. <br /> REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION:490 gallons of diesel fuel was observed in a 1000 gallon aboveground tank. David Field stated <br /> that the amount of diesel does not equal to or exceed 500 gallons. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval.
