The following is an itemized list of underground storage tank violations that have not <br /> been addressed for MARCH LANE 76 * as of February 18 , 2022 . <br /> Open violations from December 16 , 2021 inspection <br /> Violation #208 - Failure to comply with the designated operator inspection requirements . <br /> OBSERVATION : The designated operator failed to check and note all required items on the visual inspection <br /> reports . The incorrect and missing information included the following from the inspection reports : <br /> -DO report dated 3/2/2021 did not mark the mechanical float mechanisms used in UDCs box <br /> REGULATION GUIDANCE : During the visual inspection , the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report . <br /> CORRECTIVE ACTION : Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports . <br /> This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #209 as Failure to maintain a copy of the " Facility Employee Training Certificate" on -site . <br /> OBSERVATION : The Facility Employee Training Certificates dated 11 /10/2021 , 3/31 /2021 , 3/2/20213 2/3/2020 and <br /> 1 /9/2020 all contain different initial training dates . <br /> REGULATION GUIDANCE : All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1 , 2018 , or within 30 days of hire if prior to October 1 , 2018 , The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained . <br /> CORRECTIVE ACTION : Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD . <br /> MitThis violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> i <br /> Describe actions taken or will be taken to correct violation : U <br /> Violation #317 - Failure of the line leak detector to detect a 3 .0 gph leak at 10 psi and restrict or shut off <br /> flow. <br /> OBSERVATION : The 87 & 91 line leak detector failed to detect a leak when tested . <br /> REGULATION GUIDANCE : All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi . <br /> CORRECTIVE ACTION : Immediately have a properly licensed , trained , and certified contractor repair or replace the <br /> failed leak detector ( LG 113-x listed , if applicable) and obtain a permit within one business day from the EHD , if <br /> needed . If the failed leak detector can 't be replaced immediately , there is a possibility that the 87 & 91 UST system <br /> I <br /> may be red tagged to prevent fuel inputs . <br /> Page 2 of 3 <br />
