Elm Street <br /> Lodi , CA 95240 <br /> RE: SU-86-5, Parkdale Drive, 1700 feet east of Tully Road, Lockeford. <br /> A subdivision application was approved for the above premises. The following <br /> conditions imposed by the Local Health District were made a part of the approval : <br /> 1. The Subdivision shall be connected to public water and sewerage <br /> supplied by the Lockeford Community Services District. <br /> 2. The Lockeford Community Services District shall provide a letter <br /> toe the Health District, prior to recording of the Final Map, <br /> assuring the Health District that its sewer system has the <br /> capability of providing capacity to the subdivision once the <br /> proposed expansion of its treatment plant is completed, and that <br /> the sewer system will not violate the waste discharge require- <br /> ments of the Regional Water Quality Control Board. <br /> 3. The Lockeford Community Services District shall provide a letter <br /> to the Health district, prior to recording of the final map, <br /> assuring the Health District that the water supply to the sub- <br /> division will meet State requirements. <br /> 4. No lot of the proposed subdivision shall be sold, and no building <br /> permit shall be granted for any dwelling constructed in the sub- <br /> division, until sewer and water utilities are provided which are <br /> approved by the State. <br /> Since approval of the subdivision included San Joaquin Local Health District conditions, <br /> you are hereby advised that the above conditions shall be met prior to utilization of <br /> the approved land use. <br /> If the decision is appealed to the Board of Supervisors, the filing of the appeal <br /> stays the issuance of the subdivision permit or any other permits until the appeal <br /> has been decided by the Board. <br /> Please call this office for an inspection when the conditions have been met. <br /> Jogi Khanna, M.D. , MPH <br /> District Health Officer <br /> C.
