A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> NATIONAL PETROLEUM MANTECA 2072 W YOSEMITE AVE, MANTECA September 07, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 202 CCR 2712(i) Failure to retain a paper or electronic copy of the permit to operate at the facility. <br /> OBSERVATION:A copy of the UST operating permit was not found on site. <br /> REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the permit and retain it on site. <br /> This is a repeat violation, Class II. <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -DO report of 3/11/2022 and 7/28/2022 did not include the owner/operator name. <br /> -DO report of 6/2/2022, 5/5/2022,4/8/2022, 3/11/2022, 2/11/202, 1/14/2022, 12/17/2021, 11/19/2021 and 7/1/2022 <br /> left section 5 blank. Owner/operator did not state the follow up actions to appropriate compliance issues from <br /> section 3. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 10/09/2022. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($156). To schedule an appointment, please call (209)468-3420. <br /> *Note that the technician did not conduct the line leak detector for the Racing Fuel 110 Octane Tanks all other <br /> sensors tested - passed. The dispenser connected to this tank is temporarily out of service. If put in service <br /> testing must be completed as soon as possible to avoid violations. Provide 48 hours written notification to the <br /> EHD when this testing is scheduled.
