Underground Storage Tank <br /> Secondary Containment Testing Report Form <br /> 9. COMMENTS <br /> Notes: <br /> Repairs made to tank annular in both 91 STP sumps on tank bungs. Fiberglass lamination <br /> was placed around tank bungs to seal up leaks. Both 91 STP sumps were replaced with new <br /> S Bravo Systems tank sumps. All piping penetrations were replaced with NOV Fiberglass <br /> penetrations. All electrical penetrations were replaced with S Bravo penetrations. Tank <br /> annular, both STP sumps and 91 secondary lines passed witnessed testing. <br /> All tests marked "Fail"and any repairs made before or during the tightness test must be described in <br /> the COMMENTS section. <br /> Page 5 of 5 <br />
