Telephone (209) 823-8000 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00054535 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />ON 11/22/21 COMPLAINANT CONSUMED SWEET POTATO FRIES, MEDIUM WELL COOKED BURGER (MAD LOVE <br />BURGER) AT AROUND 10:30 PM. COMPLAINANT BEGAN HAVING A STOMACH ACHE AT AROUND 11:30 PM BUT <br />WENT TO SLEEP. NEXT DAY AT 9:00 AM COMPLAINANT VOMITED, AND HAD DIARRHEA AND DRY HEAVING AND <br />EYES TVVITCHING. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />#1 Demonstration of Knowledge <br />OBSERVATIONS: 4 food handler cards are expired. Renew cards in 30 days. <br />CALCODE DESCRIPTION: All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br />assigned duties. (113947) Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an <br />employee who has passed an approved food safety certification examination. (113947-113947.1) Any food handler hired after June 1, <br />2011 shall obtain a Food Handler Card within 30 days (113948). <br />#19 Consumer Advisory for Raw or Undercooked Food <br />OBSERVATIONS: Facility is serving undercooked food as per customer request and no consumer advisory provided. <br />Provide consumer advisory on site and/ or to be added to the menu in 1 week. <br />CALCODE DESCRIPTION: Ready-to-eat food containing undercooked food or raw egg and unpackaged confectioner)/ food containing <br />more than 1/2% alcohol may be served if the facility notifies the consumer. No reported illness by employees. No employees working for other food <br />facilities, day care or health care facilities. Majority of the food handler cards are valid. Staff consuming food eaten by <br />complainant. No out of the country traveling by employees. All food items on the cold hold have temp 41F or below. All food <br />items on the hot hold have temp at 135F or more. Sanitizer buckets are set up with QUAT 200 ppm in all facility stations.
