SAN JOAQUIN <br />COUNTY -- <br />Gr orpess grows he , <br />Environmental Health Department <br />Time In: 1:16 pm <br /> <br />Time Out: 1:40 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: GOLDEN OX Date: 01/10/2022 <br />Address: 410 W KETTLEMAN LN, 95240 <br />Owner/Operator: PATRONAS, IOANNIS E Telephone: (209) 333-0146 <br />Program Element: 1600- FOOD PROGRAM Complaint #: 000054581 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT ALLEGES EMPLOYEES DO NOT WEAR FACE MASKS, SILVERWARE BY THE COOKS BEING USED <br />OVER AND OVER AGAIN BEFORE BEING PROPERLY SANITIZED, COCKROACHES IN THE KITCHEN, AND OWNER <br />HAS REQUIRED WORKERS TO COME IN THAT HAVE BEEN IN CONTACT WITH PEOPLE THAT TESTED POSITIVE <br />FOR COVID-19 AND THE EMPLOYEE HAS THE EXACT SAME SYMPTOMS OF THOSE WHO TESTED POSITIVE <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION --TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />NOTES <br />I spoke to the owner who was unaware of complaint prior to my investigation. <br />I discussed the new mask mandate with the owner. <br />Owner was unaware of the new mask mandate. <br />Spatula and and cook's utensils and equipment is changed every two hours. <br />Owner does not let employees report to work if experiencing symptoms. <br />I did not observe cockroaches in the facility at this time. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
