SA N4J OAQ U I N 11 Lim& _cou Ty— <br />,,„eothess arows here <br />Environmental Health Department <br />WASTE DISCHARGE/SPILL RESPONSE <br />NOTICE TO ABATE <br />Responsible Party: SGS STOCKTON <br /> Date: Jun 25, 2021 <br />RP Mailing Address: 4863 CARPENTER ROAD Record ID#: C00GC037H <br />STOCKTON, CA 95215- Program Element: 2546 <br />Spill Location: 18129 E SOLA RD, STOCKTON <br /> Release/Spill Response (excluding Joint Team) <br />NATURE OF WASTE DISCHARGE/WASTE RELEASE: <br />An accidental mixture of sodium chlorite and etidronic acid solution caused a tank to explode during a delivery. <br />OBSERVATIONS: <br />Sodium chlorite was mistakenly added to an approximately 200 gallon black plastic tank of etidronic acid solution during <br />delivery causing an explosion of the tank. Approximately 200 gallons of liquid was released on the north side of the tank <br />and flowed into the drainage ditch. Spill site on soil covered an approximate area of 15x20 feet. The drainage ditch <br />appeared to have water in it prior to the spill. Liquid in the ditch extended approximately 50 feet. Chief Bailey and Fireman <br />Matt of the Collegeville Fire Department were on site upon arrival. Tony Ruff of Simplot on site upon arrival. PG&E <br />de-energized line from pole to pump. Area will be taped & coned off. Clean-up will take place tomorrow morning <br />(6/26/2021). Clean-up contractor is in the process of being hired. <br />Simplot to investigate and abate contamination of impacted soil and ditch. Simplot to notify SJC EHD when clean-up is <br />complete for verification and prior to backfilling. Simplot to provide a copy of disposal records for waste generated from <br />clean-up to SJC EHD and any hazardous waste determination documentation if requested. <br />VIOLATIONS: <br />Discharge of any waste, including sewage <br />(CA Health and Safety Code 5411) <br />Discharge of waste or sewage to water or within 150 ft. of waterway <br />(SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br />Improper disposal of a hazardous waste <br />(CA Health and Safety Code 25189.5) <br />Permitting or Maintaining a Nuisance <br />(SJC Ordinance Code Section 8-5100) <br />Other Violation <br />CORRECTIVE ACTIONS / ORDER: <br />Investigate and abate contamination under the oversite of the appropriate lead agency. <br />Immediately <br />''' Submit copies of hazardous waste manifests/disposal records to the EHD. <br />Within 30 days. <br />u Other Correction <br /> <br />EHD 25-01 Rev. 9/16/2020 WASTE DISCHARGE/SPILL RESPONSE NOTICE TO ABATE <br />Page 1 of 2 <br />1868 E.
