He also copied the <br />district fleet manager: Josh Maloney, District Fleet Manager, jmalone1@wm.conn, <br />Received a call from Ashan Williams (RP representative) concerning billing, <br />clarified billing information. <br />MANZO <br />06/02/2021 Reviewed narrative. Facility has an active HW program (PR0522742). MURO <br />09/20/2021 Complaint review for abatement process. Listed items on a post-it note with <br />complaint file. <br />MURO <br />09/22/2021 Emailed Josh Malone (Waste Management) <jmalone1@wm.com> to let him <br />know that I did not receive disposal records for waste generated for cleanup of the <br />MURO <br />12-09-2020 hydraulic oil spill at 11303 N 99 Frontage Rd. Requestsed that he <br />emails copies to Lead Senior Elena Manzo and copied her in the email. <br />09/24/2021 Received a copy of the final manifest via email from Eric Mankins, Waste MURO <br />Management Environmental Protection Specialist II at emankins@wm.com. <br />Manifest info: #020372734JJK for disposal of 120 Pounds of Non-RCRA <br />Hazardous Waste solids with ship pickup date 12-28-2020. <br />10/27/2021 Prepared complaint file to be abated. Marked areas in narrative that require <br />revising for typos, clarification, and/or additional information. <br />MURO <br />11/01/2021 Emailed Josh Malone (Waste Mangement ) at jmalone1@wnn.com a request for <br />status of the truck involved in the incident. Received a reply from Ashlynn Williams <br />at awilli60@wm.com stating that the truck has been repaired. <br />MURO <br />Generated site map and added to Emergency Response Record report. <br />12/21/2021 Completed complaint packet prep for abatement.
