OBSERVATION:An inspection was last done on 9/26/2018 and an inspection report was issued with one minor violation identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 10/26/2018 but was received by the EHD in greater than 30 days(on 10/28/2019) . This information has not been received timely resulting in a non-compliant status for this facility. REGULATION GUIDANCE: 25404.1.2(c)(1)A person who receives a notice to comply detailing a minor violation shall have not more than 30 days from the date of the notice to comply in which to correct any violation cited in the notice to comply. Within five working days of correcting the violation,the person cited or an authorized representative shall sign the notice to comply, certifying that any violation has been corrected, and return the notice to the UPA. CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report.
