The following is an itemized list of hazardous materials violations that have not been <br /> addressed for WEST SWEEPING SERVICES HOLDINGS LLC as of August08, <br /> 2022. <br /> Open violations from February 16, 2022 inspection <br /> Violation #6 = Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map uploaded at the time of inspection is not complete. The site map does not <br /> show/address the following required elements: <br /> 1 . Loading areas. <br /> 2. Internal roads. <br /> 3. Emergency response equipment (spill control). <br /> 4. Emergency shutoffs. <br /> 5. The symbol used to identify compressed gasses does not differentiate between the type of gasses, acetylene, <br /> propane, and oxygen . <br /> REGULATION GUIDANCE: Asite map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as "NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #10 - Failed to electronically update the HMBP within 30 days of substantial change in operations. <br /> OBSERVATION: During the inspection it was discovered an ownership change occurred January 2021 from Joe <br /> Vella to WEST SWEEPING SERVICES HOLDINGS LLC. The business has not updated and submitted the <br /> information within 30 days. <br /> REGULATION GUIDANCE: Within 30 days of one the following events, business shall electronically update the <br /> information submitted to the California Environmental Reporting System (CERS): <br /> a) A 100 percent or more increase in the quantity of a previously disclosed material. <br /> b) Any handling of a previously undisclosed hazardous materials <br /> c) A change of business or facility address <br /> d) Change business ownership <br /> e) Change of business name <br /> 0(1 ) A substantial change in the handler's operations occurs that requires modification to any portion of the <br /> business plan. <br /> (2) For the purpose of this subdivision, "substantial change" means any change in a facility that would inhibit <br /> immediate response during an emergency by either site personnel or emergency response personnel, or that could <br /> inhibit the handler's ability to comply with Section 25507, change the operational knowledge of the facility, or impede <br /> implementation of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Page 2 of 3 <br />
