Late payment of this permit was discussed with Accounts Payable in order to <br />prevent it from reoccurring. <br />Violation #203 - Failure to maintain monitoring records for 36 months and maintenance records for 60 <br />months. <br />I29This violation was corrected <br />❑Supporting documents included <br />❑ This violation will be corrected by (date): <br />Describe actions taken or will be taken to correct violation: The removal of the liquid from UDC 1/2 was documented in the DO <br />7/23/2021 report. A work order is not required for removal of liquid from UDCs however, a WO was on-site in the back-office eFile system <br />which EHD inspectors have access to. CCR 2712 allows for 36 hours to produce records and this request for a non -required <br />Rev. 9/16/2020 <br />record would meet this requirement and therefore #203 should be rescinded. <br />This is not a repeat violation as there was no liquid noted in UDC 1/2 in the August 2021 DO inspection report. <br />Page 2 of 2 <br />1868 E.
