Hammer Ln., Stockton, CA 95210 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a 1 hour plan check service request to do the following on the underground storage tank <br /> (UST) system at the above noted address: <br /> • Replace the penetrations in UDCs 112 and 11112 with split repair fittings <br /> The plan check service request has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Provide written notification to ust _.sagov.orq to arrange the following repairs and testing (a 48- <br /> hour notice is required). <br /> a. Secondary containment testing (24 hour hydrostatic or equivalent) on UDCs 112 and <br /> 11112 <br /> 4. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be <br /> issued to the installation company), or a Tank Tester's license issued to the individual <br /> by the State Water Board <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service <br /> Technician certificate issued by the International Code Council (ICC) <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced <br /> 1868 E.
