The exact location of the facility <br /> 2. The exact location of the discharge within the facility (on site location) <br /> 3. The name of the main and back-up contacts <br /> 4. The contact phone number at the facility <br /> 5. The date and time of the discharge <br /> 6. The types of materials that are or were discharged/spilled <br /> 7. Estimates of the total quantity discharged/spilled <br /> 8. The source of the discharge/spill (pipe, tank, truck) <br /> 9. The cause of the discharge/spill <br /> 10. A description of all the affected contaminated areas <br /> 11. Any damages or injuries caused by the discharge/spill <br /> 12. Actions being taken to stop, remove, contain and mitigate the effects of the <br /> discharge/spill <br /> 13. Whether or not an evacuation is warranted or needed <br /> 14. List the names of individuals and/or organizations who have been contacted. <br /> 20 <br />
