S0 JOA U 1 Environmental Health Department <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> KWIK SERV LODI BW 113* 420 W KETTLEMAN LN, LODI March 03, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The Hazardous Materials Inventory forms for propane were not accurate. The maximum daily <br /> amount cannot be less than the largest container. The largest container is reported to be 500 gallons and the <br /> maximum daily amount is reported to be 480 gallons. <br /> REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete and inaccurate. <br /> -The orientation of the underground storage tanks(UST)is inconsistent with the Environmental Health Department's <br /> (EHD)records. <br /> -The contents of LISTS are inconsistent with the EHD's records. <br /> -The map is missing locations of all emergency equipment(fire extinguishers were observed at the dispenser <br /> island) <br /> -Missing loading areas <br /> -Missing internal roads. If not applicable, list it on the map and label as"N/A". <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> FA0003776 PR0520807 SCO01 03/03/2022 <br /> EHD 19-01 Rev.12/06/2021 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
