S A N J D AR I N Environmental Health Department <br /> (L)j ;"� i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PLAZA LIQUOR#1 800 S CHEROKEE LN, LODI March 14, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 302 CCR 66262.34(d)(2) Failed to have all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: The facility is not equipped with the following equipment: materials to absorb spills and a <br /> container to place the used spill absorbent materials. <br /> REGULATION GUIDANCE: <br /> All facilities shall be equipped with the following, unless it can be demonstrated to the Department that none of the <br /> hazards posed by waste handled at the facility could require a particular kind of equipment specified below: <br /> (a)an internal communications or alarm system capable of providing immediate emergency instruction (voice or <br /> signal)to facility personnel; <br /> (b)a device, such as a telephone(immediately available at the scene of operations)or a hand-held two-way radio, <br /> capable of summoning emergency assistance from local police departments,fire departments, or State or local <br /> emergency response teams; <br /> (c)portable fire extinguishers,fire control equipment(including special extinguishing equipment, such as that using <br /> foam, inert gas, or dry chemicals), spill control equipment, and decontamination equipment; and <br /> (d)water at adequate volume and pressure to supply water hose streams, or foam producing equipment, or <br /> automatic sprinklers, or water spray systems . <br /> CORRECTIVE ACTION: Immediately obtain the following equipment: materials to absorb spills and a container to <br /> place the used absorbent. Provide a corrective action statement and supporting documentation to the San Joaquin <br /> County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 4/13/2022. <br /> Wastes generated by this facility: used fuel filters, fuel contaminated solids, fuel contaminated liquids <br /> Documents provided during inspection: list of free CUPA classes, blank hazardous waste labels, sample <br /> hazardous waste labels, Return to Compliance Certification and Corrective Action Statement <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
