A report was <br /> unsigned by the designated operator. <br /> REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, <br /> the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 <br /> hours of being provided a signed copy of the report,the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br /> issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future. <br /> This is a repeat violation, Class II. <br /> 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in under dispenser containment(UDC)7/8. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed the liquid from the UDC. Ensure that all sumps and <br /> annular spaces are maintained free of liquid. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635, 2637.2,2665, 2712 Failure to comply with overfill prevention equipment requirements. <br /> OBSERVATION: Overfill prevention testing has not been conducted and was due October 2021. <br /> REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October <br /> 13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. <br /> CORRECTIVE ACTION: Immediately schedule this testing and provide 48 hours notification to the EHD.
