The following is an itemized list of hazardous materials violations that have not been addressed for UNITED # 5447 as of March 09, 2022. Open violations from August 05, 2021 inspection Violation#6-Failed to electronically submit a site map with all required content. OBSERVATION: The site map is inaccurate.The following inacurracies were observed on site: -The water shut of valve listed along Hammer Lane was not observed -The carbon dioxide tank is listed as inside of the stockroom, but is located outside behind the store in a storage shed. -There were 2 emergency gas pump shut offs located on the outside walls of the store that were not listed on the map. -A fire extinguisher listed in the cashier area was not observed.The second fire extinguisher was observed in front of the cold box. REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". OBSERVATION: The emergency response(ER)procedures were lacking or inadequate. The listed phone number is 209-953-6200 which is SJC Office of Emergency Services.The SJC CUPA phone number is 209-468-3420.
