SAN J OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> 5 STAR MARINA 345 N YOSEMITE ST, STOCKTON March 16, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response(ER) procedures were lacking or inadequate. Section C. is not <br /> complete. The phone number for the local Unified Program Agency (UPA/CUPA) is wrongly identified as <br /> 209-953-6214 vs. 209-468-3420. <br /> REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br /> release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 8 HSC 25505(a)(4) Failed to establish and electronically submit a reasonable or an adequate training program. <br /> OBSERVATION: An Employee Training Plan has not been submitted; Employee training records were uploaded in <br /> lieu of an Employee Training Plan. <br /> REGULATION GUIDANCE: Training programs for all employees and annual training, including refresher courses, <br /> shall include familiarity with the facility's Emergency response plans and procedures in the event of a release or <br /> threatened release of a hazardous material. Training shall cover, but not limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> These training programs may take into consideration the position of each employee. <br /> CORRECTIVE ACTION: Immediately prepare or update the facility's training program, log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a repeat violation, Class 11. <br /> FA0005838 PR0529905 SCO01 03/16/2022 <br /> EHD 19-01 Rev.12/06/2021 Page 3 of 5 Hazardous Materials Program OIR <br /> 1868 E.
