Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C002QGEK4 Site Location: 15148 THORNTON RD <br />Activity Summary <br />Activity Date <br />12/10/2021 I contacted Norm Sanchez of Ancon at 916-581-9982 regarding clean-up. I <br />contacted David Weber of Premium Environmental Services at 812-706-9135 and <br />obtained the responsible party's contact information. Generated complaint record <br />in Envision. <br />Met with Norm Sanchez (Operations Manager) of Ancon Services at the site <br />location. According to the spill report, a tractor trailer's saddle tank was ruptured <br />by debris on the road causing approximately 50 to 70 gallons of diesel to release. <br />According to Norm, the diesel release impacted asphalt and two catch basins. <br />The diesel released onto asphalt, flowed into the northeast storm drain, and <br />reached the southwest catch basin. Per Norm, absorbent was applied onto the <br />impacted asphalt and swept into one 55 gallon drum. The asphalt was pressure <br />washed and the rinseate was placed into one 55 gallon drum. A total of <br />approximately 200 gallons was pumped from both catch basins and were <br />containerized. All waste generated will be left on site and will be disposed <br />appropriately, pending profiling. According to David Weber of Premium <br />Environmental Services, Western Express is the responsible party. <br />Staining was not observed on the asphalt and sheen was not observed in the <br />catch basins. Clean-up is complete. I contacted lead Garrett Backus for <br />clarification regarding notifications to other agencies and clean-up. Generated <br />inspection report. The report will be mailed to the property owner and responsible <br />party at a later time. <br />Recorded by <br />VAN G-LEE <br />(4:30pm-8pm) <br />12/13/2021 Completed photo log. Spoke with PRESTO <br />operator Tyver who provided email address for Charles Haines. <br />Entered information into the complaint record for GIS, property owner, emergency <br />response record, prop 65. Placed Prop65 into bin for mailing. <br />12/14/2021 Emailed after hours inspector from 12/10/2021 the email address for Western PRESTO <br />Express for her inspection report. <br />12/14/2021 Emailed the inspection report to chaines@westernexp.com. VANG-LEE <br />01/21/2022 Received an email from Padilla, Justin [PVV] <jpadilla@sjgov.org> asking for a MANZO <br />complaint records. Sent him an email back that I will provide it shortly. <br />01/24/2022 Created PDF file of complaint records and emailed to Padilla, Justin [PVV] MANZO <br /><jpadilla@sjgov.org>. <br />02/07/2022 Put together complaint packet to see what else was missing. Printed missing PRESTO <br />documents.
