Dave stated the spill <br />occurred at 4am, however the EHD was not immediately notified because Dave <br />was unable to make contact with PG&E's oil spill hotline who reports spills to the <br />EHD. I reminded Dave to ensure EHD is notified of spills immediately by calling <br />EH D's main line, which will dispatch to the county hospital, and the hospital will <br />contact the on-call inspector. Dave stated he cleaned up the spill and requested I <br />visit the site to verify the clean-up so that he could backfill the excavated area <br />today. Informed Melissa Nissim, CUPA Program Coodinator, of the spill. Left <br />home for site. <br />Note that the spill location was reported to the EHD by Dave Larsen, PG&E <br />representative, as 3171 McAllen Rd. However, the EHD observed the spill <br />clean-up at 3154 McAllen Rd. EHD notified Dave Larsen of the spill location <br />discrepancy. <br />Arrived on-site and met with Dave Larsen, who conducted the spill clean-up. <br />Observed the spill clean-up area- an <br />approximate 6 ft (length) x 5 ft (width) x 3 ft (depth) excavation in soil and in <br />between two bushes. Dave stated he removed part of a bush that was impacted <br />by the oil spill. No drains or waterways were observed to be impacted by the spill. <br />The spill clean-up is complete. Took photos. <br />Five 55-gallon drums of soil and a bush contaminated with oil were generated <br />from the clean-up and are being transported to the PG&E facility at 4040 West <br />Lane in Stockton for storage until disposed, per Dave (see facility PR0513829 for <br />disposal records). Dave also stated that a sample of the transformer oil will be <br />tested for PCBs. Updated Melissa on the complaint. Left the site. <br />Arrived home. Generated complaint, complaint inspection report, and photo log. <br />Began generating Emergency Response Record (ERR) form and Prop 65 (owner <br />information needed to complete ERR form and Prop 65, however owner <br />information inaccessible while away from EHD office). Emailed inspection report to <br />Alex Steele, PG&E Environmental Field Specialist.
