Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: C00053079 Site Location: 4863 E CARPENTER RD <br />Activity Summary <br />Activity Date Recorded by <br />01/13/2021 Called and spoke to John Pallen who confirmed the excavation and confirmation FLORIDO <br />sampling will be done today. John is on his way to the site. Informed John to call <br />me before the sampling is conducted so that I may go to the site to witness it. <br />John confirmed. <br />Arrived on-site and met with John Pallen and Jim Chapin. Observed additional <br />excavation. Larger excavation area is —54 ft (L) x 26 in (W) X 12 in (D). Smaller <br />excavation area is —36 in (L) x 26 in (W) x 9 in (D). Excavation is deeper than the <br />three initial confirmation soil samples. <br />Witnessed confirmation soil sampling. Three samples were taken: <br />Sample #1 taken in larger excavation area (in between samples #2 & #3) was —21 <br />inches from grade. <br />Sample #2 taken in smaller excavation area (east of sample #2) was —16 inches <br />from grade. <br />Sample #3 taken in larger excavation area (west of sample #1) was —23 inches <br />from grade. <br />Discussed excavation and sampling with Elena Manzo, Ld Sr REHS. <br />John Pallen to run each sample separately and provide the analytical results. <br />Facility is not going to backfill until analytical results have been reviewed. John <br />Pallen stated he will provide photos and the amount of waste generated. <br />01/13/2021 Reviewed sampling photos from today and from the previous sampling. FLORIDO <br />01/14/2021 Generated photo log. FLORIDO <br />Emailed the photos and photo log to John Pallen, per his request. <br />Emailed John requesting he provide photos of the waste generated yesterday. <br />Received an email from John who provided a photo of one of the lined boxes <br />containing excavated soil in it, and he states so far they have generated 4 filled <br />boxes weighing between 2,000 and 2,5000 lbs each. Emailed John reminding him <br />to provide the final hazardous waste manifests for all of the waste generated from <br />this spill clean-up. <br />01/14/2021 Emailed Elena Manzo, Ld Sr REHS, with an update on the clean-up and sampling. FLORIDO <br />01/26/2021 Emailed John Pallen asking him if he has received the results for the samples FLORIDO <br />taken on 1/13/21.
