Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: C00053654 Site Location: AUSTIN RD NORTH OF FRENCH CAMP RD <br />Activity Summary <br />Activity Date Recorded by <br />04/15/2021 Cleanup Verification site visit: MURO <br />8:44 AM En route to diesel spill cleanup. [Lead Senior Elena Manzo advised that I <br />go to the site at the start and end of cleanup]. <br />9:00 AM Arrived on site, met with Brandon Espinoza (All Environmental). Brandon <br />was on a Bobcat E50 excavator. He reported he was still waiting on the roll-off bin <br />that should arrive within half of an hour. He was smoothing out the berm and soil <br />to facilitate entry for the truck that will be bringing in the roll-off bin. <br />Discussed with Elena Manzo. <br />I told Brandon that I will be returning during the last hour of cleanup. Brandon <br />anticipated that cleanup should be completed by 11:00 AM. I let Brandon know I <br />will be returning at 10:00 AM. Asked Brandon what his plan for cleanup was. He <br />and his crew will be using the excavator to remove all contaminated soil and <br />depositing it into the 20 yard roll-off bin. I took two photos of diesel spill area. <br />9:30 AM - Left site; returned at 10:00 AM. <br />One 20-yard roll-off bin of diesel contaminated soil and oil contaminated soil was <br />generated from the cleanup. Inspected the excavated area. No oil-stained soil was <br />observed. Did not observe any diesel odor in the soil. <br />Cleanup was deemed adequate. Let Brandon know they can back-fill over the <br />excavation. <br />Brandon handed me a copy of the Castle Analytical Laboratory analytical report for <br />the two soil samples collected from the diesel and oil-contaminated soil mixture on <br />04-07-2021. Before Brandon determined that the contaminated soil cleanup <br />would require a USA notification on the day of the traffic accident, he had begun <br />shoveling the diesel and oil-contaminated soil into a 55-gallon drum. Once he <br />determined the depth of contaminated would require USA clearance, the soil was <br />dumped back onto the spill area. He collected two samples from the contaminated <br />soil he had initially excavated. <br />Left site. <br />Arrived at EHD 11:24 AM <br />Updated Elena Manzo. Discussed lab report with Elena. <br />Reviewed Castle Analytical Laboratory Lab Report. <br />Two samples were collected on the day of the incident and labeled as Flex A and <br />Flex B. Sample ID: Flex A = the first sample collected from the diesel and <br />oil-contaminated soil. Sample ID: Flex B = the second sample collected from the <br />diesel and oil-contaminated soil. <br />Sample ID: Flex A = Analyzed on 4-9-2021 for BTEX by EPA Method 8021B with <br />Report limit Multiplication Factor = 1 I Practical Quantitation Limit = Reporting Limit <br />x Report Limit Multiplication Factor <br />Analyte Benzene: <br />Reporting limit = 0.030 mg/kg <br />Report limit Multiplication Factor = 1 <br />Result: Reported as Non-Detect (ND) because analytes were not detected or <br />under the Practical Quantitation Limit. <br />Analyte Toluene: <br />Reporting limit = 0.030 mg/kg <br />Report limit Multiplication Factor = 1 <br />Result: 0.16 mg/kg <br />Analyte Ethylbenzene: <br />Reporting limit = 0.030 mg/kg <br />5105 rpt <br /> Page 4 of 7
