Spoke FLORIDO <br />to Don Beitler and Daniel Medina of Verisk3E (760-602-8700) who stated the spill <br />was due to an unknown individual siphoning gas from a vehicle in the Home Depot <br />parking lot, the spill affected only asphalt and no storm drains or soil were <br />affected, Home Depot employees applied absorbent to cleanup the spill but the <br />asphalt began to break apart, and US Ecology is en route to conduct the spill <br />cleanup with an ETA of 2:30pm. <br />Arrived on-site and observed the spill area, approximately 12 ft x 20 ft in size or <br />about the size of a parking lot space, located in the Home Depot parking lot. No <br />storm drains or soil were impacted. The spill area was blocked off from the public. <br />Met with Miranda Lazari, Home Depot Manager, who confirmed the information <br />received by Daniel and Don of Verisk3E. Home Depot generated a hazardous <br />waste drum, approximately 55-gallons in size, of <br />used absorbent from the initial clean-up. The drum is located in Home Depot's <br />hazardous waste storage area. Took photos. <br />Generated and emailed Notice To Abate to <br />- Miranda Lazari, Home Depot Manager, miranda_lazari@homedepot.com <br />- Daniel Medina, Verisk3E representative, dmedina@verisk3e.com <br />-Arcadis U.S. Inc, Home Depot's environmental contact listed in CERS, <br />Tied lAdministrator@arcadis.com <br />Updated Elena Manzo, Ld Sr REHS, and Lydia Baker, Sr RENS, on the spill for <br />possible after-hour clean-up verification. <br />Arrived at EHD. Spoke to Miranda who stated she did not know if the clean-up <br />contractor had arrived on-site, since she is no longer at the site. Miranda to <br />contact her assistant manager and let me know. Received an email from Miranda <br />stating the clean-up crew has not arrived but that the assistant manager will call <br />Lydia when the clean up is complete. Called Verisk3E and spoke to Don Black <br />who stated the clean up company caught traffic but just arrived on-site. Don was <br />unable to provide a phone number for the clean up crew on-site. Notified Lydia <br />Baker that the clean-up crew is on-site and to call Home Depot if she doesn't <br />receive a call in about one hour. <br />5105 rpt Page 1 of 2
