Do not click links or open attachments unless you recognize the <br />sender and know the content is safe. <br />Dear Ms. Baker: <br />UST Program Inspection Report <br />In respond to the Notice to Comply written during your visit of March 29, 2022, please see attached DO <br />corrections as well as the Permit to Operate. <br />Item 202: Expired Permit to Operate. See attached permit with an expiration date of 12/31/22. <br />Item 208: DO Report dated 3/21/22, correction made marking the "Mechanism used in UDC" on said report. <br />DO Report dated 11/22/21 report had conflicting dates. I reviewed the mistake with the Store <br />Manager and corrected in her presence. I was on site on 11/22/21. 1 should have noticed <br />this mistake on the same day. <br />Item 115: Partial Monitor Certification dated 12/2/2021 not in CRES. Tom has uploaded the Report today. <br />Item 308: The Oil fill sump sensor failed to activate when tested. On CGRS's final Monitor Certification <br />report they informed us (Page 5) <br />under Notes: "The Waste Oil tank is monitored by Ronan X76S Monitoring panel. The Waste Oil <br />sump sensor was replaced on <br />when repair was completed. <br />4/5/22 and passed". Please review the attached work order <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Item 110: Designated Facility signed and dated copies of Uniform Waste Manifest not on site. Missing <br />Manifest 021765660JJK. Please see <br />attached full report for Manifest dated 4/14/21. <br />Item 111: Facility did not have the exception report for Manifest 021765660JJK. See same attachment. In <br />the future I will organize these follow ups to ensure easy check for everyone. <br />Again, we can not apologize enough for the delay in submitting correction to your office.
