The following is an itemized fist of underground storage tank violations that have not <br />been addressed for United # 5446 as of August 01, 2022. <br />Open violations from March 29, 2022 inspection <br />Violation #115 - Failure to submit "Monitoring System Certification Form" within 30 days of completion of <br />the test. <br />OBSERVATION A partial monitoring system certification was performed on 12/2/2021 for service request <br />SR0083729. A test report has not been submitted to the EHD or the California Environmental Reporting System <br />(CERS). <br />REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the test. <br />CORRECTIVE ACTION: Immediately provide this report to the EHD. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #202 - Failure to retain a paper or electronic copy of the permit to operate at the facility. <br />OBSERVATION A current copy of the UST operating permit was not found on site. The most recent permit on site <br />was dated 2021. <br />REGULATION GUIDANCE: A copy of the UST operating permit and all attachments and conditions shall be <br />retained at the facility. <br />CORRECTIVE ACTION: Immediately obtain a copy of the 2022 permit and retain it on site. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #208 - Failure to comply with the designated operator inspection requirements. <br />OBSERVATION The designated operator (DO) failed to check and note all required items on the visual inspection <br />reports. The incorrect and missing information included the following from the inspection reports: <br />-The 3/21/2022 DO report did not have the box'Mechanical Float Mechanisms used in UDC's" checked off <br />-The 11/22/2021 DO report says that it was provided to the owner on 11/22/2021 but the UST Owner/Operator <br />signed the report on 11/21/2021 <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />Page 1 of 2 <br />
