NO WATERWAYS IMPACTED.
Called and spoke to Brian Hoffman of PG&E, who reported the spill to Cal OES,
and who stated Dave Larsen notified him at approximately 5:40 pm that the spill
had occurred, the clean-up was complete, and the excavated soil was in a roll-off
bin on-site. Brian stated the transformer that spilled was placarded as containing
Non PCB oil. Called and left a voice message for Dave Larsen to verify an oil
sample from the transformer will be analyzed for PCBs. EHD to visit the site on
8/15/2021 to verify clean-up is complete.
Sent Melissa Nissim, CUPA Program Coordinator, the complaint information.
Generate complaint. Emailed complaint OIR to Alex Steele, PG&E Environmental
Field Specialist.
08/15/2021 12:30pm - 2:30pm: Arrived on-site and observed a 5ft x 10 ft excavation area, with FLORIDO
a depth sloping from grade to 5 ft. The excavation had caution tape around it and
was 2-3 feet away from a pole mounting a transformer. The clean-up appeared
complete. A roll-off bin containing the excavated soil was observed on-site. Took
photos. Called and discussed the spill and clean-up with Dave Larsen, who
conducted the clean-up. Dave stated he would have an oil sample from the
transformer analyzed for PCBs tomorrow morning. Notified Dave it would be best
if he didn't backfill until we received the sample analytical results. Dave agreed.
Once the PCB results are received, will follow up with Dave on where the
excavated soil will be disposed. Left site for home. Arrived home. Generated photo
log. Documented narrative.
08/16/2021 Completed Prop 65 and Emergency Response Record Form. FLORIDO
08/16/2021 Updated Melissa Nissim, CUPA Program Coordinator, on this complaint.
