Time In: <br />Time Out: <br /> <br />trr*T.' <br /> <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: PRIME INC Date: 04/19/2021 <br />Address: 15237 N THORNTON RD, LODI <br />Owner/Operator: PILOT TRAVEL CENTERS LLC Telephone: <br />Program Element: 2546- RELEASE/SPILL RESPONSE (EXCLUDING JOINT TEAM) Complaint #: COWQJQAG8 <br />Responsible Party Contact: DAVID WHITE <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />CAL OES SPILL REPORT #21-2056: Fuel line broke releasing fuel from a saddle tank from a tractor trailer. Release went <br />onto the asphalt parking lot only. No waterways impacted. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />Arrived on-site and met with Matt Duce, Flying J General Manager. Observed the spill area impacted 5-6 tractor-trailer parking <br />spaces, located west of the Flying J truck fueling pumps. The nearby storm drain and the facility's on-site retention pond did <br />not appear to be impacted by the spill. Absorbent had been applied to part of the spill, however absorbent was not applied to <br />one area of the spill. Used absorbent was also left on the ground. The spill clean-up done by Ancon Services was incomplete, <br />and the waste generated by Ancon was not immediately found on-site. Flying J on-site personnel completed the clean-up <br />using absorbent and placed the used absorbent in a 55-gallon hazardous waste drum in the facility's hazardous waste storage <br />area. The waste generated today wil be disposed of by Flying J, a permitted hazardous waste generator. The 55-gallon drum <br />of used absorbent generated by Ancon Services was found on the west side of Flying J's aboveground fuel storage tanks. <br />Prime Inc.to provide a copy of the final hazardous waste manifest for the waste generated by Ancon Services to the EHD. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
